Huntersville Town Center, Home of Discovery Place Kids
Order Form Step 1
To place your order online using a credit
card for a brick that will be
placed at the new Town Center, Home of Discovery Place Kids,
you will need to first complete the order form below.
Once that is done, you will need to click on the submit
button. From that page you will then complete the
payment process. Orders placed that do not have a
corresponding payment will not be completed. Please be
sure to complete both steps to ensure that your mark in
Huntersville's history will be completed. Any

Orders will be accepted on a first-come, first-served basis
through January 15, 2010 February 15, 2010.
If you prefer to
pay by check instead of online with a credit card, please print out
THIS FORM, complete all required information and mail a check to the address on the form.

Please complete the form below completely.
We ask for your contact information in the event we need to
contact you. We are committed to keeping this
information private to Rotary and only use the information
collected for this fund raiser and other Rotary activities.
You will be able to opt out at anytime.
The
bricks will be laser-engraved with your personal message
EXACTLY as you type the information below. The data
entered will not be checked for spelling, etc, therefore
please be sure to review what you have typed prior to
submitting your brick donation.
You can order from one and up to four bricks
on each order. Bricks are $90 for one brick or $75 for
two - four bricks per order. Payment will be taken on
the next page after completing the information below.
